At point of renewal, you run MAP (or similar) to be able to report the total number of cores per Server - and you claim the difference in "free" cores (which you add SA to.)
We've done this process once we SQL, we still do it now. Where practical you always assign older (proc) licensing to the high core boxes, and where there is a shortfall you assign the new core licensing model to the lowest core boxes first.
Am not sure how you manage to reduce costs when at renewal/review all of this is automatically taken into account as standard. When you have a license you don't have to "fix" it to a box. When you renew or are under review nobody asks you for the specific license for a specific server, its all looked at as an overall and then the available licensing is utlised the best route.
