Lots of "depends if" ...
We would take and clean the normal AD User list and clean it up, then match user names against O365 reporting information. Its quite hard to be non-compliant with O365 when its deployed, however is easy to over-spend.
But it depends on what O365 services you have (F1 to M365 E5) and where your AD is hosted, if you have a hybrid environment or are solely cloud based.
For customers with mixed O365 environments (E1. E3, E5) they tend to set up OU to reflect this the users which is useful.

Speaking from experience, please make sure that you've checked that leavers have been correctly deprovisioned / disabled in AD so that you recoup your O365 license.
I've just finished a huge activity to recoup O365 licenses from leavers as the Joiners, Movers, Leavers process hadn't been working correctly over the past few years. Like Gemma said, that's one of the elements as to where 'over-spending' can occur.
Unless you audit your AD, you'll always think you'll need X amount of O365 licenses, when in fact you need significantly less!
