If you are starting ITAM from scratch then start with the inventory. What is out there and where is it? This may not be very easy as you most likely have multiple tools, and no tools, but start with what you know before trying to find what you don't know. Once you have that you will know what you need to find in terms of entitlement and to some extent you can see where you need to go to try to find it.
Then, as you start to identify entitlement, purchses and contracts you can start to see regional price differences, lack of volume discounts, surpluses in one region not being used to plug a gap in another, and so on, slowly building your case to move to a central procurement model.
Lastly, don't expect overnight success. $40m per year on maintenance suggests your organisation is very large so expect it take years rather than months before you get to a point where you have sufficient evidence and maturity to influence change
