I've not dealt with this metric from a Netcool perspective but did have a debate around it with regards to another product (B2B Integrator) during an IBM review a couple of years ago. Application, as a generic term, could include all aspects of an application's architecture, e.g. database, web front end, multiple servers across different environments. If you have multiple connections going to the ServiceNow application, IBM will expect you to licence all of them.
In the case of B2B, it's possible to produce a traffic summary report, which showed all the endpoints with which the program was communicating. I believe, in Netcool, you can get details of traffic flowing through an interface from the dashboard, which might give a clue on the number of endpoints that need licensing.
